Terminal dentition

Terminal dentition. Our wonderful patient suffers from chronic bruxism, resulting in severe attrition, broken teeth and loss of vertical dimension of occlusion. Preoperative CBCT reveals adequate bone in Bedrossian zones 1 and 2, as well as dense pterygomaxillary complex in zone 4. Planned for a standard PFAST.

Following the PATZI protocol, pterygoid implants Noris Pteryfit are first placed with 60+Ncm torque bilaterally. Moving anteriorly, implants are angled toward the nasal crest of the maxilla, again with 60+Ncm torque bilaterally. Strong tilted implants 60-70 Ncm torque. Maxillary CTV over 360Ncm.

Using the micronmapper and Instarisa face scan, we were able to provide 3D facially driven restorations. Prosthesis printed using Einstein 3D printer. Upper arch delivered same day with little to no adjustment on occlusion. We were able to increase and restore his vertical dimension of occlusion immediately.
